Kindergarten Health Forms

1.  History and Physical Form

This form is needed to ensure that your student is physically able to attend school. In addition to a 5-year-old physical, your child will need vaccines. According to the Wisconsin State Immunization Laws, your child will need: 4DTP/DTaP/DT, 4 polio, 3 Hepatitis B, 2 MMR, and 2 varicella vaccines (or a documented year of having had chicken pox). 

2.  Dental Form

This form is to ensure that your student’s dental health is adequate to attend school. Some dental problems can cause pain, bad breath, and trouble chewing. These things can be taken care of before coming to school, and help your student be able to pay attention and learn.

3.  Vision Form

This form is needed as vision accounts for approximately 80% of how a person learns. If not detected and treated early, vision problems in children can lead to a variety of problems. Since children often do not realize that they are not seeing as they should, and may not complain about signs of a problem (even when aware of a difficulty). Screening takes on a critical role in preventing serious problems. Untreated vision problems can lead to: loss of vision, learning difficulties, delayed sensory, motor, cognitive, and social-emotional development. Please make an appointment with an eye care practitioner and take the Vision Form with you.
