Bully Reporting Form

The Lake Mills Area School District is committed to providing a safe, positive, productive, and nurturing educational environment for all of its students. This form was created to make it as easy as possible for students and/or families to report incidents of bullying and harassment confidentially.  

• Report an incident at Lake Mills Elementary School
• Report an incident at Lake Mills Middle School
• Report an incident at Lake Mills High School

The Lake Mills Area School District takes allegations of bullying and/ or harassment seriously and will respond promptly to complaints and allegations of harassment. (Policy 5517.01)  The definition of bullying according to the Lake Mills Area School District is "deliberate or intentional behavior using word or actions, intended to cause fear, intimidation, or harm.  Bullying may be a repeated behavior and involves an imbalance of power."
