Required Documents

During the Enrollment Application, families will need to provide the following:

Proof of Residency with Name and Current Address

 Accepted documents include:

  • Current utility bill (gas, electric, cable, or city/village water)
     
  • Signed lease or rental agreement
     
  • Government correspondence (W2, cash benefits, FoodShare, housing assistance, SSI)

Please note: Cell phone contracts and driver’s licenses are not acceptable forms of residency documentation.
