Required Documents

During the Enrollment Application, families will need to provide the following:

Proof of Residency with Name and Current Address

 Accepted documents include:

• Current utility bill (gas, electric, cable, or city/village water)
   
• Signed lease or rental agreement
   
• Government correspondence (W2, cash benefits, FoodShare, housing assistance, SSI)

Please note: Cell phone contracts and driver’s licenses are not acceptable forms of residency documentation.

Birth Certificate (Certified Original)

• To request a birth certificate, visit the Wisconsin Department of Health Services.

Immunization Records

• You can access your child’s immunization record through the Wisconsin Immunization Registry.
