Pre-Registration Forms

Pre-Registration Forms only need to be completed if your student is not currently attending the Lake Mills Area School District.

Pre-Registration and Emergency Form 2026 - 1 per child

Technology Acceptable Use Policy - 1 per child

Proof of Residency Form - 1 per family

For paper copies please visit the District Office located at 120 E Lake Park Place.
