Lake Mills Elementary School 4K & 5K Enrollment
Who should complete enrollment?
- 4K-If your child turns 4 on or before September 1st
- 5K- If your child turns 5 on or before September 1st
- You are new to our school district and your child will attend 4/5-year-old kindergarten.
- You will NOT need to complete enrollment if your child currently attends our 4K program.
Where is 4K located and what days and times are 4K and 5K offered?
- Our 4K programs are located at the Methodist Church and Lake Mills Elementary School. The 4K program is half days, morning (7:55-11:10am) or afternoon (11:55-3:10pm), Monday – Thursday and no school on Friday.
- Our 5K program is located at Lake Mills Elementary and is a full day program Monday –Friday.
How do I secure an enrollment packet and schedule a time for completing enrollment?
- Please contact the LMES office at (920)648-2338 or email Danielle.Seib@lakemills.k12.wi.us, as soon as possible, to schedule an appointment.
- An enrollment packet will be mailed to approximately one week prior to your scheduled time slot.
Where/When will I return the enrollment items?
Lake Mills Elementary, 155 E Pine St, Lake Mills
- Tuesday, March 9th, 5:00-7:00pm
- Thursday, March 18th, 5:00-7:00pm
10-minute time slots are being scheduled to maintain social distancing.
- Due to COVID 19 children will not participate at this time.
What should I bring? Please bring the following items to complete the enrollment for your child:
- The enrollment packet that will be mailed to you - completed with all the required information
- Birth Certificate - The child’s birth certificate will be viewed.
- Proof of Residency - Please review proof of residency form, included in the enrollment packet, to see eligible documents.
- Immunizations - If your child received immunizations outside the state of WI, you will need to provide an immunization record.

We won't be offering a virtual option for 4K or 5K for the 2021-2022 school year. We are taking enrollments for in person instruction only.
Enrollment is completed at the District Office. Please call Roseanne Punzel for more information: 920.648.2215 x222. Once the student is enrolled, the family should contact Michelle Cook (michelle.cook@lakemills.k12.wi.us) to schedule both a mid year assessment and end of school year assessment. The student should also participate in Summer School to ensure student is ready for the next grade level.
We are planning to hold a parent presentation and student participation gather sometime over the summer, details to follow.
After an appointment for Enrollment Collection has been scheduled. Paperwork will be mailed the week prior to your enrollment collection date.
Bus transportation for ALL 4K will be free
