Permission to Administer Medication at School
This forms gives permission for school staff to administer medication at school. Please fill this form out and submit it with your child's medication to the school office. Be sure to review the Parent Letter Regarding Medication at School link above first, as some medications may need a physician's signature and some medications can be carried and self administered by students at school.
Epipen Administration Form
This form is needed for students who require an Epipen for severe allergic reactions. Parents, fill this form out instead of the Permission to Administer Medicaiton at School form as it specifies in what instance an Epipen should be used.
