Eligibility
The following must be completed and turned in one week before the start of practice to participate:

Physical Exam Form (completed by a doctor) or an Alternate Year Physical Form (completed by parent/guardian)

Authorization for Release of Medical Information

Concussion Form

SWAY Testing for Contact Sports

Sudden Cardiac Arrest Form

Participation Fee - $50 per sport ($250 cap per family per year)

All freshmen start the year with a clean slate (8th grade 4th quarter grades will not make them ineligible).
Transfer students need to set up a meeting with the athletic director to complete the eligibility questionnaire required by the WIAA.
If a student is getting a D or F when progress reports are run (week 4 or 5) they must attend athletic study hall twice a week after school for 30 minutes.
If a student fails two or more classes for the quarter they are ineligible to participate in their sport for 15 school days.
4th Quarter grades set fall start eligibility.
2 F's = Ineligible
1 F + below 1.5 GPA = Ineligible
Fall Sports Academic Ineligibility per WIAA rules is outlined on the chart below.
Athletes must maintain a 1.5 GPA.
Students must be in attendance for the entire school day to be eligible to practice or play in a game unless they are excused for an appointment or administratively approved absence.
