Welcome!
I'd like to introduce myself. My name is Toni Zastrow, and I am the LMASD School Nurse. I work 30 hours per week, and my schedule is 9:00-3:00pm Monday-Friday, splitting my time between all of the schools in the district. I am generally at Lake Mills Elementary School on Tuesdays and Thursdays but have email and voicemail for messages, questions or concerns.
Health Histories
A health history form is to be filled out by parents/guardians upon entering 4K. I review all health histories for children attending the Lake Mills Area School District and compile health concern lists for each grade level. This information is shared with staff so that we may better care for your child during their school day. If there are any changes in your child's health, please notify the office or myself as soon as possible.
Physicals
Daycare center 4K sites require physicals for incoming 4K students. These are required by law for the daycare. In addition, your child will need a physical when starting 5K along with an eye and dental exam. Forms for the 5K exams are mailed home to you at the end of 4K and are located electronically on our district health website.
Immunizations
By Wisconsin state law, the school is responsible for assuring that all student's immunizations are up to date or that a signed waiver is on record for all children within the school. Each parent/guardian is responsible for notifying the school in writing of immunizations received. By law, the school is to receive the first date of each vaccine required by the 30th day of school, the second date by the 90th day of school, and any remaining dates by the 30th day of school the following year. According to the law, failure to notify the school may result in court action and a fine of up to $25 per day of violation by the Jefferson County District Attorney's office. Illnesses
Parents/guardians are asked to inform the school if your child has been diagnosed by a healthcare practitioner with a communicable disease. A communicable disease is one that is spread from one person to another through a variety of ways that include: contact with blood and bodily fluids; breathing in an airborne virus; or by being bitten by an insect. We send emails informing parents/guardians of communicable diseases within the child's classroom and without using the child's name. Also, please remember that sick children should be kept at home until that have been 24 hours free (without use of medication) of fever, diarrhea, and vomiting. Those illnesses requiring use of antibiotics must be treated for 24hr, and be fever free prior to coming back to school. An undiagnosed rash will need to be medically evaluated prior to coming to school, as well. A note from the physician should accompany the child upon return to school to verify that the rash is not communicable.
Medication
The school district has a medication policy and a letter summarizing its contents for parents/guardians and students. Click here to view. Points to remember are:
- All medication, including prescription medications, need to be kept in the office (locked cupboard) with the medication form (except emergency medications such as inhalers). Students will come to the office to take their medication dose.
- All medication should be in the original manufacturer's container or pharmacy container when brought to school.
- No medication will be administered to students without the proper forms completed and filed in the office.
- The Lake Mills Area School District does not stock any medications in the health rooms unless parents have brought medication to the school to be used for their child.
- There is a separate medication form used for emergency planning for severe allergies - on the same website as the other medication forms.
